PORP Benevolence Fund Application

This fund is intended for members of the Place of Rest Church and the community who are experiencing financial hardship and have exhausted all other sources of support.

  1. Complete all fields

  2. Provide supporting documentations, e.g. proof of income (e.g. pay stub, EI pay stub, bank statement, etc.), rent details (contact of landlord, amount of rent etc.)

  3. Provide consent to share your information to process your application, by checking the necessary fields
